Appendix 6: Fieldwork Placement Report

The Fieldwork Placement Report is divided into 2 sections: Placement Information and Reflection Journal.

During Fieldwork Placement, students are required to maintain a journal, reflecting on their experiences regularly. Reflections should be made at the end of each shift, exploring workplace experiences.

The journal aims to foster critical review and reflection on personal beliefs, workplace experiences, and concepts from the Diploma of Community Services. Guiding questions are provided for each entry to facilitate exploration of learning progress.

For 1st placements, complete Journal Entries 1 to 5 only. For 2nd placements, complete Journal Entries 1 to 7.

Section 1: Placement Information

Section 2: Reflection Journal

Journal Entry 1: Reflect on how you applied classroom learning in your Fieldwork Placement.

Journal Entry 2: Reflect on a significant event or incident observed or experienced during your Fieldwork Placement.

Journal Entry 3: Reflect on the interaction and support received from supervisors and other agency staff during your Fieldwork Placement.

Journal Entry 4: Reflect on insights gained about working in community services through your Fieldwork Placement.

Journal Entry 5: To be completed towards the end of placement and discussed with your Liaison Officer. Reflect on the impact of the agency`s work and/or services provided to clients, regarding their identified social and cultural issues.

Journal Entry 6 (for 2nd Placement Only): Reflect on your professional development, including progress in attitude and professional skills since your 1st placement. Discuss factors contributing to your professional development and steps taken or planned towards advancing in the community welfare industry.

Journal Entry 7 (for 2nd Placement Only): As a community worker, understanding the value of self-care and self-reflection is crucial. Define self-care and provide at least 2 self-care strategies applied during this placement, discussing their personal and professional utility.

Ensure each journal entry reflects thoughtful introspection and connects experiences to learning objectives in community services.
