The student is responsible for completing this form throughout their placement. The Agency Supervisor must confirm and sign off on the student’s completed placement hours. Please be advised that recorded hours cannot exceed 8 hours per day or 37.5 hours per week. Placement must occur for a minimum of two full days per week.
