This is all about putting the medication that the patient is on together with the indication taking into account clinical appropriateness, efficacy, any adverse effects, interactions, patient factors, co-morbidities or other issues while keeping the patient central to the process.  At the end of this you may identify some Medication Therapy Problems that need to be addressed and for which you will formulate some recommendations to address these issues.
