Aim of Assignment

The aim of this assignment is to facilitate the student to reflect on and analyse a case study based on an actual patient they cared for. The purpose of the case study is to distinguish the current best available evidence to support nursing care of actual and potential problems experienced by the patient who has undergone Heart and or Lung Transplantation surgery and for the exploration of the multi-disciplinary and holistic approach to patient care.

For the purpose of this assignment please select a patient who has undergone Heart
and/ Lung Transplantation surgery that you have cared for. Please discuss your patient selection with the Staff Development Facilitator prior to completing this assignment. You must ensure that confidentiality and anonymity are upheld throughout this assignment.
