You are asked to:

• Identify a case in which there were significant prescribing challenges or errors.
• You should write a carefully anonymized description of the case in 500 words or less.
• You should identify the key issues in the case that made prescribing difficult or that contributed to the error.
• Having identified the key issues you need to reflect on what contributed to the difficulties or errors and how they should be, (or were), remediated. The issues maybe pharmacological, clinical, interpersonal, communication, organizational, etc.
• Use the opportunity of this case study to create or highlight a new approach to dealing with difficult prescribing situations or errors that is relevant to your area of special interest. In other words you should use this opportunity to create something useful for yourself as well as fulfil the requirements of the case study.
